The Challenge for My Health Guide
Like many people with a learning disability, Matthew Prosser suffered when there were changes across the multiple agencies that support him daily. New staff, new technology, new medications all require exchange of information and the loss of information could cause Matthew discomfort, anxiety, distress, or worse, medical complications and hospital admissions.
Having secured NHS innovation funding through the NHS’s SBRI program, Maldaba entered a co-production process with learning-disabled adults, their carers and families, and NHS professionals at Humber NHS Trust to innovate My Health Guide, a tablet-based app and remote sharing service for people with learning-disabilities to manage their own information daily.
My Health Guide has unlimited possibilities. It really will help put people with learning disabilities in control of their own health and wellbeing.

My Health Guide helps people manage their information, tell others how they’re feeling, and record their communications with friends, families, keyworkers and health professionals.
Users experience better quality outcomes (reduced readmissions and hospital appointments, more stable daily patterns, more effective communications) and service providers are able to give better quality support (reduced repetition in sharing information, better retention, more consistent hand-over between agencies).
Following a 12-month trial in Humber with 200 service users that saw dramatic reductions in service utilisation and high satisfaction rates across both service users and staff, Humber are scaling-up the provision of My Health Guide to 600 users and have committed to use the technology for the next 3 years.
Matthew has moved from nearly monthly hospital re-admissions, to avoiding re-admission for over 2 years.
